School Health Index: A Self-Assessment and Planning Guide

National Center for Chronic Disease Prevention and Health Promotion
Centers for Disease Control and Prevention

This self-assessment and planning guide enables schools to identify the strengths and weaknesses of school policies and programs for promoting health and safety, develop an action plan for improving student health and safety, and involve teachers, parents, students, and the community in improving school policies, programs, and services. Results from the School Health Index can help schools include health promotion activities in their overall School Improvement Plans.
