Measures and Instruments Resource Page

These resources contain measures and instruments that can be used and/or adapted by your project, as well as information on the data being collected by the National Evaluation Team.

SS/HS National Evaluation Team Surveys

These surveys were created by the SS/HS National Evaluation Team expressly for the purpose of gathering data on SS/HS programs. The NET surveys are administered by NET, which also sends each grant site a summary report about the data collected at that site. These surveys are included here to remind project directors and evaluators about the information collected by the NET (and thus does not have to be collected by projects in their other data collection activities).

Project-Level Survey

This survey collects information about new/planned policies, programs, and operations related to the SS/HS program elements. Grantees are required to participate in this survey, which is administered by NET.

School-Level Survey

This survey collects information about the nature and level of each school's involvement in planning, implementing, and supporting policies, programs, and activities related to the SS/HS program elements. Grantees are required to participate in this survey, which is administered by NET.

School-Climate Survey (Staff)

This survey collects information about perceived safety, learning environment, policies, services, and substance abuse problems at each school. Grantees are required to participate in this survey, which is administered by NET.

School Climate Survey (Students)

This survey collects information about perceived safety, learning environment, policies, services, and substance abuse problems at each school. It is optional and can be administered by grantees.

School Climate Survey (Parents)

This survey collects information about perceived safety, learning environment, policies, services, and substance abuse problems at each school. It is optional and can be administered by grantees.

Government Performance Results Act (GPRA).

Grantees are required to provide information on GPRA indicators. NET created a set of recommended GPRA items that can be used to collect this data. While providing information on the GPRA indicators is required, using these recommended items is not mandatory. However, using the recommended items will eliminate the need for grantees to develop their own questions, as well as provide consistent information across sites.
