The second in a three-part series, this webinar will provide an overview of several models of integrated care in rural communities and examples of successful integrated approaches will be discussed. The webinar will highlight an integrated care model currently being implemented in Nebraska. Information will be provided about how the featured program was developed, daily operations, and related workforce issues. Considerations for special populations, including pediatrics, will also be presented.
